As we continue in our efforts to make doing business with your clients as efficient as possible, the Investment Change form for Elite or Elite XL has now been made available as a digital eSignature form. Investment change requests can be submitted quickly and easily through the My Advisor Dashboard. The eSignature forms section on the dashboard gives you access to online forms and applications that can be signed digitally, giving you a more convenient way to work with your clients.

You will receive a copy of the form once all parties have signed. Please note that your MGA may require a copy of the completed and signed document for auditing purposes. If this applies to you, please ensure to forward the signed document to your MGA.
The following eSignature forms are already available to you on My Advisor Dashboard:
- INP-125A – Investment Change Form for Empire Life Class products and GIF contracts
- C-0170(a) – Insurance Electronic PAD Agreement
- D-0017 – Beneficiary Designation Form
- C-0050 – Transfer of Ownership Form
